What is a Medical Home?

The Medical Home model is transforming how primary care is organized and delivered. It is acarecoordinate-twocolor.jpg revolutionary way of delivering healthcare with the goal of centralizing all of your care needs by working in partnership with you, the patient, your health care providers, and your family. All care is provided in a way that is accessible, when and where you need it, and in a manner that is sensitive to your unique cultural and educational needs.

Five Components of a Medical Home

Comprehensive Care

Your Medical Home is designed to meet the majority of your medical needs, including prevention and wellness, acute care, and chronic care. A comprehensive care team works collaboratively with your primary care doctor and can include behavioral health, social work, and other specialists involved in your care. This approach reduces over medication, saves you time, and most importantly, improves your health outcomes.

As a part of Rowan Medicine, an affiliate of Virtua Medical Group, you gain access to our network of specialists who will work to develop treatment plans that ensure your overall health is taken to heart. We offer specialty care for all ages in a variety of areas, including endocrinology, infectious disease, and rheumatology. Our pulmonology department also offers support for those with respiratory diseases, in addition to at-home sleep studies. 


Patient-Centered

A key component to your Medical Home is the close relationship you develop with your primary care physician and your care team, established through respect for you as a patient, and, more importantly, as a person. This partnership allows for all decisions to be based on your unique needs, culture, values, and preferences as well as where you are on your life’s journey. You become empowered to actively manage your care and become a fully informed partner in developing your care plan. 


Coordinated Care

Managing your medical needs takes a lot of time and effort and can be extremely complex. Your Medical Home is designed to relieve you of this burden as your primary care doctor and members of your care team, including behavioral health, social workers, nurses, and medical assistants, coordinate your care across all elements of the larger health care system. This is especially important in making sure you maintain access to needed equipment and supplies, transition smoothly from different types of care, and have supports and services in your home to continue to be successful in your community. 

For our patients established in primary care, we provide a range of behavioral health services including psychiatric consultation, short-term counseling, and support for caregivers. RISN Center also offers treatment designed to improve access to preventative care including immunizations, laboratory tests,  and women’s health, all of which can be a challenge due to severe anxiety.


Accessible Services

Accessible means you get the right care at the right time. At the RISN Center, you can access your provider through the convenience of telehealth or in-person appointments with extended office hours, and around the clock either online or by phone. Through the Rowan Medicine Patient Portal, you can access your personal medical records and connect with your provider to request an appointment or referral, refill a prescription, and get a secure message to your provider’s office at any time that is convenient to you.

Another way RISN is increasing access is with our state of the art medical building that is designed to be sensory and disability friendly, and fully accessible with features like touchless door entry, Hoyer lifts in the exam rooms, adult changing tables, and family restrooms. Additional features, like on-site labs with specially-trained technicians and a sensory room for when you need a break, help calm anxiety and increase access to needed services.

The whole family can get care here, because we are located in the same building as offices for Rowan Medicine Pediatrics, Rowan Medicine Family Medicine, and the Rowan-Virtua NeuroMusculoskeletal Institute (NMI) with occupational and physical therapy services coming soon!
